Suitable hospitals, practices or specialists are approached according to the specific medical question, language and availability. The selection is explained; diagnosis and treatment remain the responsibility of the respective providers.

Appointments, communication, travel, translation and follow-up are brought together in one understandable process. You have one dedicated contact and know who is responsible for the next step.

Transition into rehabilitation

Rehabilitation is planned around goals, capacity and care setting.

After surgery, a neurological event, oncology treatment or serious illness, the next level of care must connect clinically and organisationally. We coordinate information between the acute provider, rehabilitation facility, patient and follow-up clinicians.

Readiness and goals

Treating clinicians assess stability, capacity and monitoring needs. Concrete goals follow, such as mobility, independence, swallowing, speech, graded activity or return to daily life and work.

Facility and programme

Selection considers indication, therapy intensity, medical presence, nursing needs, aids, language and admission timing. The programme is treated as a clinically justified phase of care, not a generic wellness service.

Safe transfer

Discharge summary, medication plan, wound care, mobility, infection status and fitness to travel must be clarified before transfer. If required, we coordinate suitable medical transport and direct handover to the receiving team.

Follow-up at home

Before discharge, therapy outcomes, remaining support needs, aids and review appointments are documented. The patient receives an organised handover for continued care; necessary translations and questions are coordinated.
